Existing Wording

This comment refers back to decisions made in Errata 1338. Given a proposed alternative constraint was not provided/proposed by Regenstrief in time to be included in the September errata update for C-CDA R2.1, I supported leaving the errata change as had been proposed and implemented in the US Realm header template draft C-CDA R2.1 errata release (shown below): 1. SHALL contain exactly one [1..1] code (CONF:1198-5253). a. This code SHALL specify the particular kind of document (e.g., History and Physical, Discharge Summary, Progress Note) (CONF:1198-9992). b. This code SHALL be drawn from the LOINC document type ontology (LOINC codes where SCALE = DOC) (CONF:1198-32948). However, this may become a problem for implementers who are producing the Questionnaire standard and Questionnaire response. This requirement for the LOINC code to have a SCALE of DOC will need to be taken into consideration when implementers are designing questionnaire panels (i.e. assessments) using LOINC panel codes.
