Reference should support logical references

      - add identifier : Identifier 0..1 to Reference "Business reference"
      - add language indicating that there is no expectation that a server will resolve references with just an idetifier, and a reference with both url and identifier, the url is considered correct, but the identifiers may be checked for matching safety

      Grahame will publish a version of the specification with this and will blog about it. Reference implementations and others will experiment with this. Will revisit in 1 month time.

      While in REST it generally makes sense to refer to resources by URL, in other paradigms, resources won't necessarily exist at RESTful endpoints, making URLs inappropriate. Instead, it makes sense to point to practitioners, other activity resources, etc. by business identifier. At present, the only solution is either using an extension or doing a contained reference with only an identifier - which is both heavy-weight and also fails if the referenced resource has mandatory elements that can't be supplied by the referencing system.
