Remove editor names from module sections

      Will remove the names of individual editors/contributors. Will add a ribbon at the top of the module pages similar to that present on resources and other html pages. Will explore whether it would be useful (and straightforward) to list multiple work groups for some modules (e.g. Pharmacy and PHER for Medications)

      Expectation is that these work groups will come to have ownership of their respective module pages (coordinating where there are multiple). Standards for the content of module pages in general will be set by FHIR-I and/or MnM.

      This module currently says "Editor: Josh Mandel with John Moehrke"

      We shouldn't be calling out individual's names on particular pages. We can (and should) call out responsible work groups for pages. Individuals are handled in the "credits" page. (Same issue on Terminology, ImplementationSupport Diagnostics, Financial and ClinicalReasoning and Ontology module pages)
