1. Isn't the location of the resource instance understood by the resource location (Resource.id)?

2. Is managed an indication of enforcement of the terms of the consent? If so, then this element should be expressed as enforcment, and would need to be 0..*. Where zero indicates that it is not yet agreed to by an enforcing organization, and many might be responsibile for enforcing.

3. Is managed an indication of the organization which captured the consent onbehalf of a group of organizations. Such as is done with an Health Information Exchange (HIE) that authorizes a set of participants (sometimes all participants) in the exchange to capture consent directive authorizing the whole of the HIE actions under some DURSA.
