It is unclear what the distinction between Condition and Observation is. There's too much overlap between the two. Is the fact that someone is nauseous a condition or observation? I tend to look at "Problem List" as a list of things that the clinician wishes to track about the patient. If condition is meant to be that, then perhaps renaming it to Concern or something like that might help.
