1)* CarePlan.activity.outcome (Backbone Element) 0..* *Identifies the outcome at the point when the status of the activity is assessed. The activity outcome is independent of the outcome of the related goal(s). For example, if the goal is to achieve a target body weight of 150 lb and an activity is defined to diet, then the activity outcome would be calories consumed whereas the goal outcome is an observation for the actual body weight measured. Note that this should not duplicate the activity status (e.g. completed or in progress).

      2) *CarePlan.activity.outcome.result (CodeableConcept | Reference(Any) ) 0..1 *Details of the outcome or action resulting from the activity. Can be a reference to an "event" resource, such as Procedure or Encounter or Observation, which is the result/outcome of the activity requested/planned. The requested/planned activity can be defined using CarePlan.activity.detail OR using the CarePlan.activity.reference (often a reference to a "request" resource).

      Note:

      CarePlan.activity.outcome.resultCodeableConcept (formally known as CarePlan.activity.outcome)

      CarePlan.activity.outcome.resultReference (formally known as CarePlan.activity.actionResulting)

      3) Goal.outcome (Backbone Element) 0.. *Identifies the change (or lack of change) at the point when the status of the goal is assessed. The goal outcome is independent of the outcome of the related activities. For example, if the Goal is to achieve a target body weight of 150 lb and a care plan activity is defined to diet, then the care plan's activity outcome would be calories consumed whereas goal outcome is an observation for the actual body weight measured. Note that this should not duplicate the goal status (e.g. met or not met).

      4) *Goal.outcome.result (CodeableConcept | Reference(Observation)) 0..1 *Details of what's changed (or not changed)

      Comment:

It is unclear what the new outcome field of the Careplan.activity section should contain. Can this be further defined in the definitions?
