Move entered-in-error to be at the same level as active and inactive in the value set http://hl7-fhir.github.io/valueset-allergy-intolerance-status.html

(The mixing of the clinical and workflow status will be discussed as part of http://gforge.hl7.org/gf/project/fhir/tracker/?action=TrackerItemEdit&tracker_item_id=11151 )

      Comment:

AllergyIntolerance.status code set placed "entered-in-error" as a child of "inactive"

This is the result of (1) mixing clinical status and workflow status into one single concept, (2) erroneous grouping

"entered-in-error" should be removed from the "inactive" hierarchy in the code set

Also need to align with "Condition" resource to avoid inconsistency

      Summary:

entered-in-error is not "inactive", align with Condition
