There are lots of variations on each major vital sign. And LOINC has them pre-coordinated (Not usually the best way). Think the best way to deal with them is as additional related observations as in the following LOINC panel (code 34566-0). Would consider the addition of these following variables -

8457-6 Blood pressure method,

8358-4 Blood pressure device Cuff size,

8359-2 Peripheral artery measurement site,

8326-1 Type of Body temperature device,

8327-9 Body temperature measurement site,

8361-8 Body position with respect to gravity,

8352-7 Clothing worn during measure.
