Existing Wording: For illustrative purposes, the following diagram depicts a simple use case of a patient accessing their personal health record (PHR) enabled by an underlying electronic medical record (EMR) system. The EMR plays the role of the FHIR server in this example.

Proposed Wording: For illustrative purposes, the following diagram depicts a simple use case of a patient accessing their own health record enabled by an underlying electronic medical record (EMR) system. The EMR plays the role of the FHIR server in this example.

      Comment:

"Personal health record (PHR)" should not be used. That is commonly known to be something different than just the patient's record within an EHR/EMR. A "patient portal" typically provides access to that patient's EHR data, but a "PHR" is generally viewed as owned/controlled and maintained by the patient.
