      Proposed Wording: Requires relationships between various concepts: Health Concerns, Goals, Interventions, Outcomes

      Comment:

      "Problems" are a subset of Health Concerns for Care Plans, and not a separate section, so they should not be listed as requiring relationships between various acts. Also, it should not say "various acts" because not all of the things listed are acts (e.g., goal and outcome are observations). I realize the text is straight copied straight from C-CDA, but I think it's wrong there too!
