      Rather than attempting to model a patient's "arrival time" by detecting different statuses of encounters that may be modeled differently by different systems, an EpisodeOfCare could be used to relate the encounters unambiguously. This would provide a clear definition for "arrival time", which is at the heart of the need to determine observation status from a quality measurement perspective.

      We will add EpisodeOfCare reference to the QI-Core-Encounter profile to capture this information. In addition, we will either define a binding that adds an Observation status to the Category binding (currently ActEncounterCode), or work with US-Core to determine whether that is an appropriate extension in that space.

      Existing Wording: Code = SS

      Display = short stay

      Definition = An encounter where the patient is admitted to a health care facility for a predetermined length of time, usually less than 24 hours.

      Comment:

      Does this ActEncounterCode cover patients who are in "observation status"? Today, we do not have a good way to model patients who are in observation status, in that they may be physically located in the ED or in an inpatient bed, but they are not ED or Inpatient patients. Many times, a patient may initially be an observation patient, who then becomes an inpatient admission, or could be in the ED > Observation > admitted to Inpatient.

      Enhance the definition of "short stay" or SS to include patients who are in "observation status" or create a new Code specifically for patients who are in "observation status".
