The invariant "contained.where(('#'+id in %resource.descendants().reference).not()).empty()" doesn't handle situations such as Media referencing a contained Binary resource by content/url, nor does it handle a narrative that points to a contained PDF with <image src="#whatever"/>. Both should be supported. (Though the latter is going to be trickier for FHIRPath to manage right now.)
