      The ConceptMap.group.unmapped.url is used to provide a reference to another concept map resource to use for the source code system/code when the group.target.equivalence code has no entry in the mappings, as is documented for the ConceptMap.group.unmapped.mode element and the conceptmap-unmapped-mode value set.

      Note that Unmapped does not include unmatched, and we should clarify the definition of unmapped to make clear that if an concept has a mapping that is 'unmatched' then processing does not proceeed into the unmapped section. Add a section explaining the use of unmapped

      The second url description is 'canonical URL for other concept map' - this is unmapped.url - so how can an unmapped concept have a canonical URL for the (a) map? Is this a refernce to another concept map resource to use for the source code system/code when the group.target.equivalence code is equal unmatched??
