The Patient(s) need to have their required and participation *status *set too, so would have another backbone element for patient that looks the same as participation. So it was collapsed in the same structure. There is a specific search parameter for patient which enables simple searching for the patient explicitly. Group appointments need the multiple patients, so has multiple cardinality.
