      Instant doesn't allow expression of any information that can't be expressed using dateTime. Profiles can constrain dateTime to be the same as instant if they wish. No change is necessary to meet the submitter's use-case.

      If the medication is administered using a device, the MedicationAdministation.effective timestamp will be autogenerated by the device. Therefore one of the Observation.effectvie choices should be "instant" for UTC date/time.
