Concept Map http://build.fhir.org/conceptmap.html Section 4.1.1, 4th paragraph
Existing text: Each mapping for a concept from the source to target system.
However, the definitions of the codes in concept-map-equivanence http://hl7.org/fhir/2018May/valueset-concept-map-equivalence.html read from target to source: Existing text for "wider": The target mapping is wider in meaning than the source concept.
The text does not consistently define whether the mapping direction is from source to target, or target to source. Direction is important - in general (80%) maps are from source to target, and not from target to source.
Example of the difference: Diabetes Type 2 with Chronic Kidney Disease narrower than than Diabetes Type 2 (assuming map is from source to target) versus Diabetes Type 2 with Chronic Kidney Disease wider than Diabetes Type 2 (assuming map is from target to source).
The documentation should be more clear as to the map direction.
