      Systems generally support the simple concepts as defined by FHIR and are very interoperable. The additional extended values that you've included would go into an extension on the property, selecting the "other" value in the core element.

      Comment:

      The assignment or selection of gender raises a question as to whether these 4 categories are adequate. Should there be a category for transgender-Male to Female and Female to Male as these changes are a reality. As to preferred gender-I am not sure that clinically that outweighs the actual gender and should be covered in a social history comment/narrative used to inform caring for the individual patient. Suggested additional categories: TF or FT (define as Transgender originally Female and TM or MT define as Transgender originally Male and allow extensions to see what gets used and if it should be added as a category
