      "Note that the CodeSystem resource and ValueSet.compose offer no support for defining hierarchies and groups, but this does not exclude servers from using extensions or other knowledge to introduce such groups as an implementation feature". Discussion will continue with the intent of clarifying this in R5. However:

      Add an extension on Valueset.compose.include that defines an abstract grouper and the concepts (or other groupers) that it groups. These will become abstract contains entries when the server expands the value set. Group can just be text, but can also include a code tied to an underlying code system resource. These groupers are not part of the value set and will not appear in a flat expansion

      Existing Wording: An expansion may include entries in the expansion that only serve an arbitrary grouping purpose, to make it easier for a human to use the list. These entries have no system or code, and must be marked as abstract.
