      There's no need for "other" when they're optional elements. Making these extensible would require changing to full CodeableConcept elements which would significantly increase the complexity of the data types. In v2 and v3 and most other specifications, these notions are structural. The boundary of "home/work" are not about time of day, but more about the appropriateness of what circumstances are appropriate to use a particular identifier.

      Why aren't use and system extensible Codings? We've already identified the need for "other" system values; there are surely "other" use values as well.

      The classifications of use are arbitrary. Are home/work more useful than day/evening? Does it matter that a number is mobile or not, or is the key issue "when can I get the person"? I have a work cell phone. Is that work, or mobile, and why is that decision better than the other way?

      I would rather see extensible codings for both. At a minimum we should allow "other" for use.
