We've done a lot of talking and have a lot of pieces (Condition, Observation, Linkage, List, EpisodeOfCare) but we don't have a particular place in the specification that talks about how they all fit together and are intended to be used. Some guidance on how all of this maps to CCDA's notion of Concern would be good too.
