Goal.status value set has codes for:
Sustaining = The goal has been met, but ongoing activity is needed to sustain the goal objective.
Achieved = The goal has been met and no further action is needed.
However, we are missing an element to convey (before the goal is met) whether the goal is a one time goal (after met, status is achieved) versus an on-going goal (after met, status is achieved). This is needed so a system can programmatically know which status (achieved or sustaining) is appropriate upon finding an Observation showing the goal is met.
Examples:
* Goals to get an immunization and/or complete training might be a one-time goal (when met, it is achieved)
* Goals to maintain HbA1c levels or blood pressure or weight might be an on-going goal (when met, still need to sustain)
