However the potential values that could be in the extended set is very likely to be either too fine or too coarse for others, so expect that jurisdictions may decide to use their own set, but retain the simple set easing interoperability in most cases.

      Why do we not have "Transgender" in administrative-gender value set?

      The scenario is if the patient is transgender then how to record? The most suitable option would be "Other" but will not be specific.

      I would highly recommend to also have transgender in the category so that we can avoid fragmentation. Otherwise, the systems who want transgender in their system they will make extensions and custom profiles. My concern is that rely as much as we can on the FHIR spec and do not promote making custom profiles so that health data in the world should be in the same interoperable format. Thank you.
