Having title and description as separate data elements is very common in PDMS and we are facing the problem of representing that in FHIR. Title is often used e.g. for UI display in lists, so it would be useful to have a (short) title for the document in addition to the description.

A title is also specific for the single document instance and therefor not the same as DocumentReference.type.
