Clarify VSD-3 for ValueSet reference

      Add a note to the Composition rules clarifying explicitly the meaning of having (valueset+system+filters) or (valueset+system+concept)

      Note: the way it works is specified by implicition in the Composition rules already but additional clarity is useful.

      As for all three elements in one include vs 3 includes. This is already described by the existing composition rules:
      * Multiple include statements are cumulative - e.g. the value set contains the union of all the includes
      * Within an include, all the criterion apply -e.g. the value set contains the intersection of the criterion

      ValueSet.compose.include has three elements: concept, filter and valueSet. VSD-3 states that ValueSet.compose.include "Cannot have both concept and filter".

      The question is if a valuset has all three elements (concept, filter and valueSet), should there be three include?
