This example: "An anti-prescription: recording an instruction not to take a medication" is no longer good because there's a way to do this in core. Replace this with "A flag on a [Patient.contact] indicating they are not to be contacted - i.e. a next of kin for record-keeping purposes only"

We'll also need to fix the XML and JSON instance example and the sample warning message graphic and surrounding text.

The example showing narrative would probably be best replaced with one for the COndition family history modifier.
