Update the questionnaire-choiceColumn extension to add a column that says "forDisplay" of type boolean 0..1 with a definition "If true, indicates that this column should be used as (or as part of) the display value for code or reference"

comments "For codes, only one column can be picked and it must be a column that corresponds to a legal display name for the code from the value set. For references, multiple columns can be picked. If more than one column is chosen, the Reference.display will contain concatenate the column values in the order defined separating them with a single space."

There is a bullet under 8.1.2 that reads, "When an initialExpression ... will return a resource, backbone element or complex data type, the questionnaire-choiceColumn extension can be used to control which data elements should be exposed to the user (and what labels and widths should be allocated to each element so exposed)."

However, earlier, initialExpression is said "to cause the initial answer for the question to be set to the specified expression". Since it is for one question, it is hard to see how there could be more than one value being shown to the user as the answer to question, or what role labels and column widths could have. I thought choiceColumn was for answer lists (in a table fomat), before user selected a choice, not for data that is going into the answer's form field.
