The status tracking is moved from the appointment to the encounter when the status is moved to "fulfilled" which in most cases indicates that the concounter is in progress. To clarify this, the description of the appointment status fulfilled code will be changed from: "This appointment has completed and may have resulted in an encounter." to "The planning stages of the appointment are now complete, the encounter resource will exist and will track further status changes. Note that an encounter may exist before the appointment status is fulfilled for many reasons"
