In the SDC IG, it incorrectly raises the following error:
QuestionnaireResponse/questionnaireresponse-sdc-example-ussg-fht-answers: QuestionnaireResponse.item.item.item.item.item error Item has answer, even though it is not enabled (item id = '2.1.1.4.1.1.1')

Grahame's comments:

the problem is that the questionnaire evaluator ignores context, and picks the wrong item (which I did also when I reviewed it manually). I'm going to have to completely re-write that part of the validator, and it's not happening this week
