There are two labels in the diagram in this section that end with "Extract QuestionnaireResponse", which sounds like a QuestionnaireResponse is be extracted from something, but what I think is meant is that the data extraction process is being run on the QuesitonnaireResponse to extract data from it into other resources. This would be more clear if the labels read "Extract from QuestionnaireResponse" instead.
