Practiioner.identifier required elements different than Patient.identifier elements

      Please clarify why Patient.identifier requires .type, .system and .value but Practitioner.identifer only requires .value. It seems like knowing the type of provider ID is important. Given that Practitioner.identifier isn't required (0..1), it seems like there is an "out" for systems that can't provide a system and type (although I suspect most will be able to).
