As well, a BSeR specific value set for PractitionerRole.code would be helpful. The values in the currently linked value set may not apply to a referral recipient. And a binding of Required means that it can't be expanded.

The Structure Definition for the Referral Initiator is incorrect so I can't do a comparison between this profile and that one, but how are they different (other than possibly the .code value)? Can these be combined into a single profile?
