On the table of contents page, there are two entries for "Questionnaire Launch Context" (numbered 12.44 and 12.45), one right after the other. If you click on them, it seems that one is talking about the code system, and one is talking about the value set. It would be better to differentiate the entries in the TOC.

This also happens for the two subsequent entries (both "Questionnaire Entry Mode").
