Remove the 'prompt' code from the itemControl code system

The description for entryFormat says, "Additional instructions for the user to guide their input (i.e. a human readable version of a regular expression like "nnn-nnn-nnn"). In most UIs this is the placeholder ..."

The description for code prompt (itemControl) says, "Text is displayed immediately below or within the answer-entry area of the containing question item (typically as a guide for what to enter)".

If these are not accomplishing the same pupose, some clarification is needed. Otherwise, maybe code prompt should be dropped?
