      The element might not have been 'populated' at all - or might have been populated by a different mechanism. If there is an answer when the user is done filling out the questionnaire, and the query returns no results, then the data should result in a create.

      Will clarify the language as follows:

      When peforming an extraction, the query is executed. If a record is found, the data in the QuestionnaireResponse will be used to update that record. If the query returns no results, the data in the QuestionnaireResponse will be used to create a new record.

      Seems contradictionary - no elements - no data - no extracted data to use. What is the required behavior? To create an empty element?
