      For security reasons, HL7 implementation guides prohibit HTML with embedded controls, which would be essential to displaying a form. That's why we only use a PDF.

      Suggest a HTML page, in addition to the PDF, could be shown on the Example Questionnaire web page.

      Suggest to present a statement indicating the interface displayed in the 'companion PDF' may appear differently in different web browsers.
