      The order is defined by the order of the elements in the instance. Implementations of FHIR SHALL retain the order of all elements in arrays. That's a non-negotiable part of FHIR. (If systems didn't do that, then first names and middle names would be confused, address lines would be confused and lots of other things would break.) Adding an additional extension would convey no new information and would create a chance for inconsistency. Is systems need to add elements into their database to ensure that array order is retained, they're free to do so. However, the array order is sufficient for exchange purposes.

      Add a Question Order Field as an Extension to the Questionnaire Resource

      Purpose: To display Questions on the User Interface as per the ingested numerical order (i.e. 1st, 2nd....) from a FHIR database.

      Explanation: In the Base Questionnaire Resource, there is no field that maintains the order of the questions (as received from the FHIR database) to be displayed on the User Interface. The proposed extension will persist the data (i.e. Item) in the same order it was received from the external entities (for example: from the Assessment Centre or from a Lime Survey database - via an API). This is because, if we do not maintain the order of the questions as received from the external entities, the default is to consider the first row persisted in the database as the first question and then display it accordingly. This might not always be the case.
