Without visiting other pages in the spec, it's not clear what this field contains and there's no explanatory text on this page (unlike for template). I think that this field should contain a url to, for example, the ServiceRequest that a clinician is in the process of ordering and that was provided to the payer's CDS service by the EHR in the context element of the CDS Hooks request. This begs the question – is this url intended to point to the EHR's FHIR server or the payer's? If payer, the payer will be persisting and sharing a FHIR resource that isn't yet available RESTfully on the EHR's FHIR server and that likely contains PHI.
