Existing Wording: MessageHeader.focus.reference: BSeR Referral Request
Proposed Wording: MessageHeader.focus.reference: BSeR Referral Task
Comment:
BSeR implementation guide is explicit in that it covers a referral workflow for extra-clinical services. In section 12.6.2 of the FHIR specificaiton, the management of a workflow state is described as follows: "The workflow state is represented by the Task resource. The Task state machine [...] shows the use of Task.status to represent the general infrastructure state of the Task resource. [...] The combination of Task.status, Task.statusReason and Task.businessStatus provide the mechanism for tracking the workflow state."
http://hl7.org/fhir/workflow-communications.html#12.6.2
Furthermore, the messaging pattern for workflow management is clear that the focal point of the MessageHeader must be a Task resource.

Summary:
Use the Task resource to share the state of the referral. The Task.basedOn reference will point to the ServiceRequest for the referral.
