We will drop the code search parameter and add:

questionnaire-code (Questionnaire.code)
item-code (Questionnaire.item.code)
combo-code (Questionnaire.code, Questionnaire.item.code)

That way searchers can search on exactly what they want. (Don't want to retain 'code' as it would most naturally apply to 'questionnaire', but that's not how it's historically been used, and the name is ambivalent.)

The search parameter "code" for Questionnaire only allows searching on Questionnaire.item.code. It should also allow searching on Quesitonniare.code.
