Support use of ExampleScenario with other standards by making instance.resourceType a preferred binding

      Will change the data type to be Coding rather than code, will expand the value set to include the v2 message structures and will loosen the binding to be extensible rather than required.  Will also update the language in the specification to make it clear that 'resource' is not being used in the FHIR sense, but rather in the sense of a data object instance being shared.  (The structure might be FHIR or something else entirely).  The contained objects might be of a different type than the base type.  Will update the descriptions to refer to "instance" rather than resource where appropriate and remove any language that suggests this is FHIR specific

      If we want to use EampleScenario with other standards - DICOM, a V2 message... then the resourceType should not be a Required binding.

      Suggestion: should be extensible binding.
