The description of item.definition (https://www.hl7.org/fhir/questionnaire-definitions.html#Questionnaire.item.definition) mentions a field "options", which went away in R4. It was replaced by answerValueSet, which is also mentioned in the same description, so the obsolete one just needs to be removed. I suspect it was just an oversight in editing.
