Proposed Wording: There should be a separate section in this (and all other uses cases addressed in this IG) that describe the specific actors involved in this use case. In this case, it seems the care coordination is being mediated by the payer; the payer receives clinical information from provider, and the payer uses that information and/or makes it available to other providers, for care coordination purposes.

In the Examples section, in addition to a 'true' care coordination example, the IG also includes an example of gathering the information for HEDIS/Stars quality program. That is NOT care coordination, and there is another use case in the IG associated with quality. Recommend moving that example out of this use case.
