It is possible that ClinicalImpression is a better match for CancerDiseaseStatus than Observation, but Observation is part of every FHIR interface, while ClinicalImpression is not widely implemented, and far less mature. Neither Argonaut nor US Core, Epic or Cerner implement ClinicalImpression. Rather than going the "right in theory, wrong in practice" route, no change is recommended.

Suggested resolution: Considered - no change.

CancerDiseaseStatus seems to fit the definition of ClinicalImpression better than Observation. Consider changing the base resource.
