      a) As per Graham Grieve's recommendations, we intend to use Diagnostic Report as the root and Observations as the section headers. Based on feedback from the next ballot we may revisit this.

      b) This is an artifact of the CIMPL logical model. For the next ballot we will not be using ActionRequest and we currently intend to use ServiceRequest and/or Appointment.

      c) We intend to add the patient history section tro the home page figure, and for the next ballot will research profiling Procedure for inclusion. Currently we consider MedicationStatement out of scope but may reconsider in the future.

The approach of using Composition with defined sections makes sense. I'm not sure what benefit using DiagnosticReport brings, just the Composition may be sufficient however, it's difficult to assess this completely in the absence of the logical models underlying the IG.

Why is a new ActionRequest resource being defined? Can ServiceRequest not be used with the appropriate intent? If ActionRequest is the best pathforward, will other resources (such as ServiceRequest or Appointment) be update to be able to point back to ActionRequest to indicate the fulfillment of the recommendation? If ServiceRequest can't be used, a quick discussion of the rationale may be useful in the IG.

The overview figure on the Home page doesn't include the patient history section that is specified as part of the Composition profile. It probably should. Nor does it include any indication that profiled MedicationStatement and Procedure resources can be part of the report.
