It's difficult to asses the overall organization of the IG when profiles are missing, unlinked or only generically defined.
For Composition.relatesTo.targetReference, what is the obf-Composition reference? Should this be breastrad-BreastRadiologyDocument? If obf-Composition is a valid profile, the hyperlink should work.
The profiles page doesn't seem to include the Conditions profile (http://hl7.org/fhir/us/breast-radiology/2019SEP/StructureDefinition-obf-Condition.html). Nor does it contain the ActionRequest profile (http://hl7.org/fhir/us/breast-radiology/2019SEP/StructureDefinition-obf-ActionRequest.html), Procedure (http://hl7.org/fhir/us/breast-radiology/2019SEP/StructureDefinition-obf-Procedure.html) or MedicationStatement profile (http://hl7.org/fhir/us/breast-radiology/2019SEP/StructureDefinition-obf-MedicationStatement.html). I think these should be listed there they can be part of the Composition and related resources.
The obf-observation profile (http://hl7.org/fhir/us/breast-radiology/2019SEP/StructureDefinition-obf-Observation.html) which presumably holds the Patient Risk observations is not well defined. Additional narrative text (if not constraints) are necessary to understand how to use this profile.
