Agreed add discussion of pathway vs workflow:

Patient state is concerned only with the past and current state of the patient; things that actually happened

Plan is concerned with things that are planned or proposed to happen; what we want to do or are considering

Workflow is concerned with how the things that we plan to do are actually accomplished

      Recommendation vs Intervention

      Recommendations don't necessarily imply interventions

Intervention starts to get into the particular implementation strategy and represents a method for the delivery of a recommendation

Consider adding a topic to the approach documentation that discusses pathway versus workflow

Add a diagram illustrating the overall scope of the representation of a pathway in terms of a set of 3 concentric circles. In the innermost circle is patient/case state, in the middle circle is plan/decision state, and in the outermost circle is workflow state
