      The latest Argonaut spec (http://build.fhir.org/ig/Healthedata1/Argo-DSTU2/StructureDefinition-argo-diagnosticreport.html) explicitly states: "When lab test or lab panel, such as CBC, is ordered, a DiagnosticReport represents the order fulfillment and references each of the resulting discrete Observations within that panel. Each Observation resource represents an individual laboratory test and result value, a "nested" panel (such as a microbial susceptibility panel) which references other observations, or rarely a laboratory test with component result values."

      This is consistent with the way mCODE has approached the problem, namely, DiagnosticReport at the top level and Observations or nested Observations below that.

      Regardless, based on the approved ballot resolution in GF#24531, we will be removing the CBC and CMP laboratory profiles. In its place, we will list the LOINC codes of interest to mCODE, instruct implementers for the time being to follow US Core Laboratory Result profiles, and await publication of CIMI lab profiles.

      Need to get clarification about how the Argonaut group conceives of how to use DiagnosticReport. I had understood that it can contain observations and ordinary observations can include nesting, so it might be that the spec is OK for carrying CBC with Auto Diff. However, you may be on to something that should be corrected in the Argonaut profile and other panels. I will ask around and hope to get an answer before the meeting.
