      While the US X12 standard allows for the specification of unique-within-priorauth identifiers for detail lines, we have not found this pattern to be utilized within any other country. Currently this is handled as an extension and should this pattern in the future be found to predominate then this may be promoted to an element in the base resource.

      Sadly, this seems to imply that Claim items should be resources. Option 1: (preferred) Reference by sequence. Also (more specific to claim and claim response), why use sequence in an array. Arrays have natural orders. You could simply use those.

      Option 2: Alternatively, allow item.identifier 0..*and remove item.sequence 1..1. The latter is not essential in an ordered list to provide "sequencing", and is described as "Necessary to provide a mechanism to link to items from within the claim and within the adjudication details of the ClaimResponse." Since you don't find it sufficient, and do find it necessary to include any number of identifiers, this seems to incdicate that claim itself is broken.

      Summary:
