      The specification does not define how to use it in a clinical context, neither do the underlying FHIR versions.

      E.g. most PlanDefinitions depend on triggers. When the trigger is activated, the PlanDefinition is applied.

      How does this integrate in clinical practice? Questions related to this include: 1 Does this happen in the background and is the clinician notified when this has happened? 2 Does the specification assume that this happens directly when the triggers are fired?
