      The language in the specification refers to "rejected due to insufficient information" which is a different concept than "denial". In the case of being rejected due to insufficient information, a new prior authorization can be submitted with the needed information for processing. If the authorization request is denied, then out-of-band appeal processes still apply.

      We will add language clarifying that this specification does not cover the process of appeals as it is not handled by the X12 278 transaction. Providers should consult the payer in question to determine the appropriate appeals mechanism.

      2.3.5 appears to be referring to a denial case. However, in denials there is an appeals process that happens for the existing request. A new request is not generally submitted as described here.
